Coordinated Care

In the context of RITMOCORE coordinated care is referred to as the feasible relationships between Hospitals and Primary Care to improve quality of life of patient and saving scarce resources for the health-care system. Focus is on increasing quality of care and efficiency by right information sharing and comprehensive care path implementation.

Relevant functionalities are:

  • To strengthen the direct communication between relevant professionals, at all levels of a care pathway, with appropriate multi-channel services, exploring the different schemes already present in the consortium. Some of our participating Hospitals are already fully integrated with Primary Care while others include, in their reference area, several different Primary Care services providers without hierarchical relationships and non-interoperable Information Systems.
  • To make available for clinicians the remotely monitored data from the pacemakers and other monitoring devices into the patient home.
